The Elkhart County Health Department has added three (3) additional walk-in clinic dates in December/January for school aged children only Kindergarten through 12th grade, Medicaid or no insurance only.

The Immunization Clinic will be opened from 8:30 a.m. to 11:00 a.m and 1:00 p.m. to 3:00 p.m. Doors will close early when clinic is full. The clinic is located at 608 Oakland Av. Elkhart, IN 46516 and the cost is $10 per shot.
YOU MUST:
- Bring all immunization records with you or child will not be seen.
- Child must be accompanied by parent or an adult (18 yrs or older) with written permission from the parent.
- Guardian must present legal paperwork.
