Physicals for 2025-2026 will need to be dated on or after April1, 2025. We will not accept physicals before this date for the upcoming year. Any physical dated for March of this year will need to be resubmitted with an April 1 date.
Physicals completed after April 1 are required to be on file with the middle school prior to participation in athletics during the next school year.
We are only accepting Physicals on the Final Forms this upcoming year. Parents/Guardians MUST fill out the Final Forms.
